Most governance failures aren't failures of mechanism. They're failures of attention.
The DAOs that function have a small, accountable core doing the work, a transparent way to challenge it, and a culture that treats voting as a last resort rather than a first one.
Everything else — quadratic this, conviction that — is a detail on top of those fundamentals.
